The Patient Centered Medical Home concept is an innovative setting in health care currently being proposed to area practitioners. This initiative fosters highly involved relationships between not only individual patients and their personal physicians, but it also involves the patient’s family when fitting. The Patient Centered Medical Home takes advantage of available resources such as registries, health information technology and health information exchange among participating practitioners to ensure that patients will receive the medical care they require. While switching over to the Patient Centered Medical Home concept is still a work in progress, many physicians are embracing this change and actively seeking ways to implement this process into their own practices. Not only does this method promote better health care provided for patients, it also promotes efficient use of resources and technology which ultimately cuts expenses immensely. While receptive practitioners are at varying stages concerning the progress of implementation, MiHIA is set on helping all willing participates reach the goal or restructuring their respective practices into a more proficient and more profitable venture. MiHIA is currently researching sources of funding to assist with the costs associated with the technological advance of the Patient Centered Medical Home and is also more than willing to help with the actual implementation and training at each respective practice.
