Participation in the series will enable you to catalog your organization's population health efforts, see the balance of your current activities within and across portfolios, and identify areas you may wish to focus on to accelerate your population health improvement efforts. At the end of the series, organizations will have an Action Plan to support the regional acceleration of population health improvement efforts.
